Mediclaim forms and Investment Undertaking / PAN

IMPORTANT NOTICE

Dear All BIS employees,

Please submit the Mediclaim forms, copy is available with concerned team leader of each team.

Also in order to avoid tax deductions please fill the investment undertaking form, and in case you do not have a PAN card please apply for the same asap.

The undertaking form is also available with concerned TL of respective department . In case not able to procure from TL, please mail me at ars@bird.in and I will send you the required form.

Every employee's salary slip has "medical" amount and you are requested to submit the medical bills in support of that amount before th 15th of every month. This is also useful to avoid tax deductions at source.

In case of any ambiguity please call me @ 0124 2342075. (10 am to 5 pm - Monday or Saturday) to seek clarifications.
